ALL NEW STUDENTS:

A student enrolling in any of the Silver Valley Unified School District schools must have the following documentation prior to enrolling:

  • Birth Certificate or Passport
  • Immunization Record
  • Unofficial Transcript for high school students or recent report card for K-8 grade students
  • The names and phone numbers of 2 emergency contacts (Commander or 1st Sgt acceptable)
  • Parents of Special Education students please bring the IEP or 504 Plan for enrollment
  •  Inter-district Transfer paperwork if you reside outside of the district boundaries
  • Physical for Kindergarten or 1st Grade students
  • Dental Assessment Screening for Kindergarten or 1st Grade students

 KINDERGARTEN / FIRST GRADE:

  • Physical Requirements (call 380-3124 for appointment)

Entering Kindergartners: Physical completed within 6 months PRIOR to the first day of school

Entering First Graders: Physical completed within 18 months PRIOR to the first day of school.

The Child Health and Disability Prevention Program is designed to promote the good health of children. California law requires that every child have a health check-up (physical) within 6 months prior to entering kindergarten. For entrance into kindergarten or first grade, the parent must present a record of a completed physical.

Oral Health Assessment for all entering Kindergartners:

All Kindergartners must have an oral health assessment completed 12 months prior to the start of school. This can be completed by a dentist or dental hygienist. 

  • Proof of age for Kindergarten – must be 5 years of age on or before December 2 of the school year – i.e. birth certificate, passport, immigration certificate

  • Inter-District Transfer Form

IF YOU LIVE IN THE SILVER VALLEY UNIFIED SCHOOL DISTRICT, BUT OUTSIDE OF Fort Irwin SCHOOL ZONE:

  • Intra-District Transfer Form

COMPLETED REGISTRATION PACKET AND EMERGENCY CARD:

  • Red Emergency Card, completed and signed
  • Home Language Survey
  • Permission Form (for TV, media, etc.)
  • After School Release form

If you do not have all of these documents with you, the registration cannot be completed. Classes will NOT be assigned at registration.
