Our mission is to ensure enrollment in the Defense Enrollment Eligibility Reporting System (DEERS), and provide Common Access Cards (CAC) and/or Official Identification Cards to Soldiers, Family Members, Retirees as well as Civilian and Contract Employees working on Fort Irwin/& Surrounding Area. Online Appointments can be made for Family Members, Retirees, DOD contractors/civilians (CAC); and Disabled Veterans. Appointments are not available for Regimental Soldiers; they will be serviced at BLDG 184 (Regimental S-1), unless they require services for their Family Members (i.e ID card or DEERS Enrollment) or the Soldier is In/Out Processing.

Walk-ins are always very welcomed; they are served on a first come, first serve basis (appointments are priority). The wait times can exceed 1-2 hours on any given day. To avoid an excessive wait, please schedule an appointment.
ID Cards can be replaced when they are within 60 days from the expiration date.

DOCUMENTATION
ALL customers who are age 21 and older must have two (unexpired) forms of ID to receive a new ID card. One must be a state/government issued photo ID. Some acceptable forms of identification include: Military ID Card, Driver's License, Photo ID issued by Federal, State or Local Government, Passport, School ID with Photograph, SSN Card, Native American Tribal Documents, Voters Registration Card, Birth Certificate, Hospital Record, or School Record. Documents must be original. True certified copies of the originals are also accepted; All Foreign documents must be originals with an Apostille and translated in English.
ALL court documentation must be state or county certified stating that it has been filed and recorded in the courthouse
SSNs will no longer be printed on ID cards and should not be relied on as a unique ID number. These changes will occur when the cardholders' expired cards are renewed. ID cards can be renewed 60 days prior to the expiration date.

ELIGIBILITY REQUIRMENTS
Military Members
· Official documentation must be presented for any updates or changes (i.e. Promotion Orders, Active Duty Orders, Name Change Memorandum, Medicare Card or Official Notification Letter from Social Security Administration, etc)
· Must present two (2) valid forms of identification (primary must be photo ID).
Family Members
Must have two (2) valid, unexpired forms of identification (primary must be photo ID) and one of the following:
· Sponsor present
· Valid Special Power of Attorney (POA)
· DD Form 1172 (ID Card Application) signed by the Sponsor and a Verifying Official of the DEERS office
Full-Time Students (21-23 years old) – Attending accredited Institution
Sponsor must be providing over 50% support – must be stated in block 89 of the DD 1172
Must have two (2) valid, unexpired forms of identification (I-9 Lists of Acceptable Documents above)
Letter from Student Registrar’s Office or National Clearing House (Enrollment Verification Certificate) that states the following:
· Full- Time Student Status
· Obtaining associate’s degree or higher
· Anticipated date of graduation
· If student falls below full-time status, the ID card must be turned in to a local ID card facility.
any medical costs incurred will be the responsibility of the Sponsor
DOD Civilian
· Must have DD 1172-2; signed by the CPAC office or HR Assistant
· Must have two (2) valid, unexpired forms of identification
DOD Contractors
· All Contractor Employees must be processed through the Contractor Verification System (CVS) to obtain a CAC
· If going overseas, a Letter of Authorization (LOA) must be presented reflecting this information , for initial card LOA cannot be older than 30 days
· Must have two (2) valid, unexpired forms of identification
** ALL EMERGENCY ESSENTIAL CIVILIANS ARE RQUIRED TO PRESENT A DD FORM 2365 (OVERSEAS EMERGENCY ESSENTIAL POSITION AGREEMENT) **

INITIAL ENROLLMENT OF FAMILY MEMBERS
Spouse
· Original Marriage Certificate
· Birth Certificate (Must contain the name of at least one parent)
· SS Card or other official document showing social security number (See Documentation section above)
· Photo ID issued by state or local government
- Final Divorce Decree, annulment or death certificate (if Sponsor or Spouse has a previous marriage)
Newborn/Children (If parents are not married, see Illegitimate Child section below)
· Birth Certificate or Hospital Proof of Birth must contain the name of the Sponsor
· Social Security Card (if received); if not a TIN (Temporary Identification Number) will be generated by the RAPIDS system
An individual with a TIN has three 90-day grace periods to acquire an SSN or their Direct Care at a MTF (Military Treatment Facility) may be suspended per AFI 1.24.2

· Court order granting custody to the Sponsor. Court order must stipulate that the child will be in the care and custody of the Sponsor for no less than twelve (12) consecutive months. The court order can designate length of custody by age, time or permanency.

PLEASE NOTE:
Children receive an ID card at the age 10 unless DUAL Military or Single Parent. Other conditions may apply, i.e. the child’s temporary guardian lives away from the parent’s duty station and must use a medical facility other than the one keeping the child’s records; or the child resides temporarily away from the sponsor and there is no eligible spouse.

FAMILY SERVICE MEMBERS GROUP LIFE INSURANCE
Like SGLI coverage, FSGLI coverage is automatic and the maximum coverage is the automatic level. However, spouses must be enrolled or registered in DEERS for proper premiums to be paid.
Premiums for spousal coverage are due the month of the event that causes coverage to occur (e.g. Nov 1, 2001, induction of the Service member into service or the date of a marriage while in service). Premiums start and will be automatically deducted from the Soldier’s military pay when they enroll their spouse in DEERS. Failure to enroll the new spouse in DEERS will result in a debt that will be collected retroactively to date of marriage or entrance into the military whichever is later.
Dual Military:
The same applies to Service Member married to Service Member, they also must register each other as a spouse in their respective DEERS records. FSGLI coverage is automatic and the maximum coverage is the automatic level. Premiums start and will be automatically deducted from the Soldier’s military pay when they register their spouse in DEERS. Failure to register each other will result in a debt that will be collected retroactively to date of marriage or entrance into the military whichever is later.
The spouse's birth date in DEERS determines the premium rate to be collected. If the spouse's birth date is incorrect in DEERS, the incorrect premium rate will be collected. And if the spouse's birth date is not in DEERS, premiums will be collected at the maximum rate.

DEPENDENCY PACKETS
DFAS Dependency Packets are NOT submitted through the DEERS/ID Card Office, nor can we assist in completing the paperwork. We do have a guidance sheet available in our Office. The following website denotes where the paperwork can be sent and who should be contacted should additional questions arise. ONCE APPROVED:
Parents must bring in Photo ID, SSN, DD 1172, DD 27-88 and birth certificate of Sponsor
Parent-in-law must bring in Photo ID, SSN, DD 1172, and DD 27-88. Marriage certificate of Sponsor and Spouse along with birth certificate of Spouse
Incapacitated child photo ID, SSN, DD1172, DD 27-88, medical sufficiency statement (memorandum from doctor)
Ward must bring in birth certificate, SSN, (Photo ID if over 18 yrs of age), DD 1172 and DD 27-88
NOTE: Dependency Package must be recertified every 4 years

LOST OR STOLEN ID CARDS
Two (2) acceptable unexpired forms of ID (primary must be photo ID) from the I-9 Lists of Acceptable documents along with the documentation below are required to receive a replacement ID Card for ALL Lost or Stolen ID Cards:
Soldiers in the rank of SSG (E-6) and below must provide a counseling statement (DA Form 4856) from their immediate supervisor. When a Soldier loses their CAC card for the second time, the Soldier must then provide a counseling statement from either the Commander of First Sergeant. The Soldier must provide two forms of ID for identification purposes, to the ID card facility.
Soldiers in the rank of SFC (E-7) or above must submit a sworn statement (DA Form 2823) to the ID Card facility stating the facts of the lost ID Card which must state when and where ID Card was last in the Soldier’s possession and location it may have been lost. When Soldiers in the rank of SFC (E-7) or above lose their CAC for the second time, they must provide a counseling statement (DA Form 4856) from the Commander or First Sergeant. The Soldier must provide two forms of ID for identification purposes, to the ID card facility.
DOD/DA Civilian Employee and DOD Contractor Personnel will be required to complete a Lost ID Card Statement provided by the ID facility at the time of re- issuance of the ID card describing the date, place and circumstances of the loss or theft. The Statement must be signed by their CAC Sponsor (immediate Supervisor), Card Holder & the SSM on site. MISCELLANEOUS INFORMATION
IN- PROCESSING: Soldiers only need to in process the ID card Section if they require a new CAC, PIN reset or additional ID Tags; if none of the listed items are required, the checklist will be signed off by the In Processing Personnel in BLDG 561.
OUT-PROCESSING (ETS, CHAPTER, OR RETIREMENT): Must have CAC, DD Form 214 WORKSHEET, Clearing Papers and an Escort (if applicable).
PLEASE ENSURE TO SIGN YOUR FINAL DD 214 PRIOR TO CLEARING THE ID CARD SECTION.
REGIMENT SOLDIERS: PLEASE UTILIZE BUILDING 184 (REGIMENTAL S-1) FOR YOUR DEERS/ID CARD SERVICES WITH THE FOLLOWING EXCEPTIONS è INPROCESSING, OUTPROCESSING, DEPENDENT ID CARDS
